Case Manager for Homeless Veterans, Veterans Leadership Program of Western Pennsylvania - Pittsburgh, PA

Mission:  Veterans Leadership Program (VLP) of Western Pennsylvania provides essential housing, employment, and other vital supportive services to eligible veterans, service members, and their families to improve self-sufficiency, sustainability, and quality of life.

Description:  Clients participating in this program are typically single adult male or female veterans who are parents of children under the age of 17. The Case Manager will provide outreach and case management services to homeless veterans and their families in Allegheny County and will ensure that comprehensive services are provided to each client to assist them in accessing recovery programs, employment opportunities, housing assistance, and supportive services.  The long-term goal of this program is to promote greater stability and self-determination for single homeless veterans and their children, which will have a positive impact on the community as a whole.  Recordkeeping is also an important part of the position--keeping client and agency files accurate, up-to-date, and in order is a critical function.

Requirements:
·  Positive attitude with effective oral and written communications skills are a must.
·  Computer literate: familiarity with Microsoft Office (Excel, Outlook, Powerpoint, Word) and relationship management database software.
·  Act 33 and Act 34 Clearances.
·  Experience with recovery (drug & alcohol) programs.
·  Must have a reliable vehicle.
·  Previous experience working in human services, particularly with persons who are homeless, is preferred.
·  Veteran with honorable discharge is preferred.
·  Two or more years of case management experience.  Bachelors Degree or related experience in field.

Responsibilities:
·  Write client background summaries.
·  Provide comprehensive case management services to each client.
·  Prepare an Individual Service Plan (ISP) for each client.
·  Conduct one face-to-face office visit per month with each client.
·  Conduct one face-to-face home visit per month with each client.
·  Prepare and present a variety of topics at monthly group meetings, including: life skills, financial literacy, social readjustment and reintegration, drug and alcohol dependency, peer support, job retention, and others determined by client needs.
·  Complete reports in an accurate, thorough, and timely fashion.
·  Maintain clear and up-to-date progress notes for each client.
·  Coordinate with other VLP staff to conduct outreach activities throughout the regional community.
·  Represent the Agency at public forums and community events.
·  Implement and enforce program rules and regulations as set forth by the federal Department of Housing and Urban Development (HUD).
·  Other duties may arise and be assigned by the Program Director.
