Reporting to the Director of Program Services, The Social Work Manager will be responsible for ensuring that the profession's unique psycho social perspective is carried through an Interdisciplinary care model. Through the program wide management of the department, will ensure that the expectations are met relating to social assessments, planning, interventions, eligibility, recertification and ensuring that the appropriate care is delivered in order to optimize the health status, quality of life and independence for our Participants. Successful candidate must work well in a fast paced, variable work environment with multiple locations.
