The Chronic Disease Self-Management Program (CDSMP) is a lay-lead participant education program offered in communities throughout the US and several other countries.  Participants are adults experiencing chronic health conditions such as hypertension, arthritis, heart disease, stroke, lung disease and diabetes.  The program provides information and teaches practical skills on managing chronic health problems, as well as giving people the confidence and motivation they need to manage the challenges of living with a chronic health condition.

The Project Coordinator is responsible for managing the day to day operations of the project including collaborative efforts among partner organizations, recruitment and retention of peer leaders and older adult participants, ensuring that the project is executed in accordance with the project proposal, work plan, budget and CDSMP model fidelity expectations.  This is a contract position which is scheduled to be funded for a two year period.  Additional responsibilities will include:

*Refining and further developing the project work plan within the first month of project start and maintaining as a living document to meet project outcomes and deliverables.
*Monitoring work plan progress relative to timeline and budget and reporting variances.
*Executing the project communication plan.
*Maintaining an active and supportive role in recruitment and retention of peer leaders and older adult participants.
*Planning and executing meetings and events including those outlined in the project work plan and additionally as needed to support the achievement of project outputs and outcomes.
*Monitoring model fidelity according to Stanford Chronic Disease Self-Management Program implementation manual.
*Capturing and disseminating Lessons Learned throughout the life cycle of the project in pursuit of recruitment and CDSMP delivery targets.
*Tracking program and individual performance objectives and routinely reporting on progress and outcomes according to federal, state and local reporting needs.
*Participating in weekly staff meetings and consistently following-up on action items.
*Engaging in proactive risk management including mitigation strategies and contingency planning.

Bachelor’s degree from an accredited college/university in a related field and/or demonstrable Project or Program Management experience that spans the project lifecycle from initiation to closing is required.  Strong verbal and written communication skills, strong relationship building skills and the ability to manage multiple responsibilities, along with a proficiency in creating, reading and analyzing budgets and financial reporting documents is key.  A valid driver’s license and access to a reliable vehicle are needed for this position will involve local travel within Allegheny County.
